Shareholders of record should expect to receive an IRS Form 1099-DIV from Broadridge Corporate Issuer Solutions, CareTrust REIT’s 2016 dividend paying agent. “Street name” shareholders should expect to receive the IRS Form 1099-DIV from their applicable bank, brokerage firm, or other nominee.
