Our patient care philosophy is simple: The health of our patients is our priority. We work with top-quality providers, but we also have a unique perspective. We believe the job of managing patient health goes far beyond the clinic. We support whole-patient wellness, with a focus on prevention and full-cycle care of their conditions. Our coordinated care teams work with patients and their providers to manage follow-up and preventive care. We also provide a full range of preventative care tools and information for patients to manage their long-term health and help them connect with local support organizations and resources.
We help providers focus more on what they love most about their profession: helping patients, and less on administrative burdens. We do this by surrounding providers in our network with a full support team that helps coordinate and manage patient follow-up care to help them be proactive in patient outcomes. We have many options for provider engagement that also include support with practice administration, insurance plan relations and outreach support.
We help to bridge the gap between the insurance plan, the provider and the patient to ensure patients efficiently understand and utilize the services available to them. We engage with our plan partners to support network, utilization management, quality management, care management, medical management, claims processing and programs to improve quality star metrics.
We believe broker and Medicare advisor partners, are essential leaders in our patients’ healthcare journeys. We support brokers by helping patients at the clinic level understand and take advantage of their plan benefits. We also offer a variety of broker community events throughout the year, as well as training, resources and individual support from market leadership.
